    • $40 per person (drinks are included)
    • $20 for children ages 3 to 12
    • each dojo please
      • submit one rsvp form
      • pay with one check (bring it to the party)
      • donate one door prize, value $50 or more
    • no charge for advisor, cabinet, or guest (please be sure to to list on form)
